Level 1 – $495

At home visit by a physician for a non-acute medical condition for one patient. Physician will diagnostic patient current issue, address a course of action. Minimum follow-up required.

Example: Approximate time of visit: 30 minutes

Level 2 – $600

At home visit by a physician for a non-server complicated issue. Would require physician to provided medication or IV and could involve the ordering of labs or imaging. Will required follow up by phone or telemedicine for proper communication.

Example: Approximate time of visit: 30–45 minutes

Level 3 – $800

At home visit by a physician for a complicated issue. Would require physician to provided multiple prescribe of medication or IV and could involve the ordering of labs or imaging. May require referral to specialist. Referral to E/R maybe be required. Will required multiple follow up by phone or telemedicine for proper communication.

Example: Approximate time of visit: 60+ minutes

Phone Consultation – $275

A quick and simple way to get a few of your questions answered by a Physician. Follow-up may be suggestion if physician feels need more information in person.

Telemedicine Video Consultation – $325

When not able to meet in person, a physician interaction over the web. Discuss on going medical issues on a secure video call. Diagnose and evaluations made base on sight and may require a follow-up in person.

COVID-19 Testing – $200

SARS-Covid-19 Rapid testing available for at home convenience. Rapid and accurate Covid-19 Testing. *PCR and Antibody testing available upon request.
